You are invited to complete the profile questionnaire at right and submit it by the reply date on the reverse of this brochure.  An editor will be assigned to your profile, although you will be the final editor of your unabbreviated entry since you will see a proof prior to publication.  You also are invited to submit a photograph with your profile that will accompany your biography.

Purchase of this volume is not a requirement for inclusion, however, you are eligible for prepublication discounts as a biographee.
